Dr. O'Neill has an active basic and translational research program and is also director of the ultrasonography program in the Renal Division. His NIH-sponsored research is in the area of vascular calcification, specifically on the role of endogenous pyrophosphate in the etiology of vascular calcification. Dr. O'Neill's clinical interest is in renal imaging and interventional nephrology. He directs the ultrasound program in the Renal Division, which he initiated in 1994, and directs numerous courses for nephrologists at Emory and at national and international meetings. He is past-president and founding member of the American Society of Diagnostic and Interventional Nephrology.

Current research focuses on the pathophysiology of vascular calcification in renal failure with particular emphasis on the role of endogenous calcification inhibitors such as pyrophosphate and matrix Gla protein. In addition, the role of vitamin D in vascular calcification is being investigated. Potential therapies based on the central role of pyrophosphate are being developed and tested, including pyrophosphate analogs and novel inhibitors of alkaline phosphatase. Studies are performed on animal models and on an in vitro vessel culture system developed in this laboratory.

Dr. O'Neill directs the ultrasonography service in the renal division. The service performs diagnostic studies of kidneys, renal transplants, and urinary bladders, as well as guidance for percutaneous renal biopsies. This program has been in operation since 1994 and currently over 1200 studies are performed per year. Dr. O'Neill has directed CME courses for outside nephrologists since 1997. Information on these courses can be found on the education page of the Renal Division. This clinical area has also been the focus of research relating to imaging in renal disease.
